  4. Pensioner card having photograph of the applicant.
  5. Driving License.
  6. Certificate of identity in Original signed by a Member of Legislative Assembly.
  7. Election Card / Voter ID.
  8. Certificate of identity in Original signed by a Municipal Councilor.
  9. Ration card having photograph of the applicant.
  10. Ex-Servicemen Contributory Health Scheme photo card.
  11. Aadhaar Card.
  12. Certificate of identity in Original signed by a Member of Parliament.
  13. Passport.
  14. Certificate of identity in Original signed by a Gazetted officer.

  8. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.

  4. 10ᵗʰ or 12ᵗʰ Mark Sheet of recognized Board.
  5. Pension payment order.
  6. Matriculation certificate.
  7. Domicile certificate issued by the Government.
  8. Passport.
  9. Affidavit sworn before a magistrate stating the date of birth.
  10. Marriage Certificate.
  11. Central Government Health Scheme Card.

Is PAN Card Reprint required?
Financial Institutions or Banks request a PAN Card as proof of PAN. If your PAN Card is stolen or misplaced, you can apply for PAN Card Reprint and present Reprint PAN Card.
Who has to sign the Reprint PAN Application?
The PAN Card Reprint Form must be signed by the PAN Holder. If he or she is unable to sign, a left thumb impression must be done on the PanReprint form.
