  5. Certificate of identity in Original signed by a Member of Parliament.

  8. Pensioner card having photograph of the applicant.
  9. Certificate of identity in Original signed by a Gazetted officer.
  10. Certificate of identity in Original signed by a Member of Legislative Assembly.
  11. Certificate of identity in Original signed by a Municipal Councilor.
  12. Election Card / Voter ID.
  13. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
  14. Ration card having photograph of the applicant.
  15. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.

  1. Marriage Certificate.
  2. Birth certificate issued by the Municipal Authority or any office authorised to issue birth.

Is PAN Card Duplicate Copy required?
Financial Institutions or Banks ask to give PAN Card as PAN proof. If PAN Card is not present with you or not found, you can apply for PAN Card Duplicate Copy and present Duplicate PAN Card.
