Pan Card for below 5 years is called Child PAN Card. Child PAN Card do not have Applicant's Photo on it. It has parent's/guardian's signature on it. For Child PAN Card following documents are required:

1. Aadhaar card

2. Birth certificate issued by the Municipal Authority or any office authorised to issue birth

4. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking

1. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
2. Election Card / Voter ID.
3. Certificate of identity in Original signed by a Member of Legislative Assembly.

5. Pensioner card having photograph of the applicant.

7. Ex-Servicemen Contributory Health Scheme photo card.

11. Depository account statement less than 3 months old.
12. Consumer gas connection card or book or piped gas bill.

14. Landline/Broadband connection bill less than 3 months old.

16. Aadhaar Card.
17. Credit card statement less than 3 months old.
18. Election Card / Voter ID.
19. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.
20. Property Registration Document.
21. Bank account statement less than 3 months old.
22. Passport of Spouse (Husband/Wife).

Any one of Child's Parent or Guardian needs to sign the PAN Card Form. If he/she is unable to do sign then left thumb impression is required on form.

Child PAN Card will be delivered to Parent or Guardian's Address by Courier mentioned in PAN Form while applying pan card for Child online. Address proof having same address needs to be submitted along with application.
